Tampa Dental Wellness offers an online self-booking tool powered by a third-party patient portal (Archy). By using the booking portal, you acknowledge and agree that:
The submission of a booking request does not guarantee a confirmed appointment until you receive written or verbal confirmation from our office.
You are responsible for providing accurate and complete information, including your name, contact details, preferred location, and reason for visit.
Appointment availability is subject to change without prior notice.
4.2 Cancellation and No-Show Policy
We ask that you provide at least 24 hours’ notice if you need to cancel or reschedule your appointment. Repeated no-shows or last-minute cancellations without notice may result in a cancellation fee or refusal of future online bookings at our discretion.
4.3 Minors
Patients under the age of 18 must be accompanied by a parent or legal guardian during all appointments. Appointments may not be booked on behalf of a minor by anyone other than a parent or legal guardian.
SMS and Electronic Communications
5.1 SMS Consent
By checking the SMS consent box on any form on this Site, you expressly consent to receive conversational SMS text messages from Tampa Dental Wellness. These messages may relate to:

By proceeding with a financing application or payment plan, you agree to be bound by the terms of the financing provider in addition to these Terms. Tampa Dental Wellness is not responsible for decisions made by third-party financing companies.
All fees for dental services rendered are due in accordance with the payment terms communicated at the time of service. We accept most major insurance plans; however, it remains the patient’s responsibility to understand and verify their coverage.
